How to register

We have an open list, and welcome requests from patients living in our practice area. You can register by bringing your NHS medical card or completing a form available from reception. If you do not have your NHS medical card, then you may be asked to bring other photo ID when you attend, ie Passport or driving licence, this ensures that we have the correct information to send to the Business Services Authority. When you register you are classed as being registered with the practice, however, you can request to see a particular doctor for an appointment. Upon registering you will be offered an appointment with a Health Care Assistant for a health check if you are over the age of 16 years. If you are under 16 years you are only required to complete a registration form.
